

The following information is provided by the Office of Student Affairs and Services. I. Prematriculation and Orientation Week 2008
A full week of Prematriculation and Orientation activities begins on Saturday, August 16 and continues through Sunday, August 24. All entering students are required to attend the entire week of activities. We encourage you to relocate to East Lansing by August 11.
During Prematriculation and Orientation Week you will have the opportunity to get to know your classmates and the faculty and staff in the College. Activities will include a day of team building exercises, comprehensive orientation to the College including the Block I curriculum and preparation for your academic success; seminars on financial aid, student health services and insurance, and immunizations. Classes will begin at 8:00 am on Monday, August 25.
We will post the Prematriculation and Orientation week schedule as soon as it's available.
II. Basic Life Support (BLS) Healthcare Provider Course
All students must complete the BLS for Healthcare Providers training for certification. This certification is required. Sessions will be held during Prematriculation and Orientation Week. You do not need to register in advance for this training. Successful completion of the course will provide you with a certification valid for two years. Cost for this course will be posted later this spring.
If you are certified through August 2009, please bring your certification card to the Office of Student Affairs and Services when you arrive in East Lansing so that we can document your compliance with the requirement.

III. Matriculation and White Coat Ceremony
Prematriculation and Orientation Week concludes with a Matriculation and White Coat Ceremony at 3:00 pm, Sunday, August 24, 2008 at the Wharton Center on the campus of Michigan State University. Parents, spouses, family members, and friends are invited to attend. Formal invitations for your family and friends will be included in a packet of material that will be mailed in mid-July. A reception immediately following the ceremony will offer students and guests an opportunity to meet the Dean of the College as well as other faculty, administrators, staff, and students.

One important component of this very special ceremony is the presentation of a preclinical white coat to each student. You must complete and return the White Coat Order Form to ensure that we have the correct size coat for you. The deadline to return this form is Friday, June 6, 2008.
Please note that the file you submit is NOT a PDF form; it is an FDF data file through email, and then we import your data into the form. Please don't make any changes to the file name or extension before sending it, as this could cause problems with the data import.

Troubleshooting
If you are unable to submit your form using the Submit button, the problem is typically with your email client. Note that these solutions only apply if you do not already have a default email client set up. If you already have a default mail client set up, you should just need to click the SEND button in your email program.
To submit the form with a desktop email application (such as Outlook, Outlook Express, Windows Mail, Thunderbird, Eudora):
After you've filled in the form, click the Submit button on the PDF form.
In the Select Email Client dialog box, select Desktop Email Application; then click OK.
In the Send Data File dialog box, click Print Form if you want a copy of the filled-in form; then click Send Data File. Your default email application displays a new email message with the To, Subject, Body, and Attachment fields automatically filled in.
Use your email application to send the email.
Click Close in the Email Confirmation dialog box in Acrobat.
To submit the form with a web-based email service (Gmail, Hotmail, Yahoo!Mail, etc.):
Click the Submit button on the PDF form. If the form fields are blank, the Email A Blank Copy Of This Form dialog box appears; click Email A Blank Copy.
In the Select Email Client dialog box, select Internet Email; then click OK.
In the Sending The Data File dialog box, click Save Data File.
In the Save Data File dialog box, choose a location on your computer to save the file; then click Save.
Open a new browser window, log in to your web-based email service, and use your service to create a new blank email.
In the Sending the Data File dialog box in Acrobat, select the value in the To field; then right-click (Windows) or Control-click (Mac OS) and choose Copy.
In your blank email message in your Internet email service, click in the To field, and paste the data you copied. Repeat steps 6 and 7 for the Subject and Message Text fields.
Use your Internet email service to attach the data file (that you saved in step 4) to your email message.
If you want a copy of the filled-in form, click Print Form in the Sending The Data File dialog box in Acrobat.
Click Close in the Sending The Data File dialog box.
Note that the form is compatible with Adobe Reader 5.0 and higher. If you need to update your version of Reader, click here to go Adobe's website and download the most recent version of Reader. Please note that a few people have also had trouble submitting the form from computers using the Windows Vista operating system.
If you are unable to submit the form by email using the SUBMIT button, print your completed form and fax it to Milissa Moryc at (517) 432-1051.
IV. Immunizations
All CHM students are required to have their vaccination records on file with the University Physician's Office. Click HERE to go to the Health Care Professions Students website. Scroll down to the bottom of the page and click on the link to the HCP Immunization Form for complete information and instructions for submitting your vaccination records.
The East Lansing and Lansing communities offer a variety of housing options, from subsidized apartment complexes to townhouses and condominiums, most within a few minutes' drive from the MSU campus. Other convenient, surrounding communities include Okemos, Haslett, Holt, Mason, Waverly, and Grand Ledge. Arrangements for fall housing should be made as soon as possible. Please try to arrange housing so that you may move in by August 11, 2008. There is very little time available during Prematriculation and Orientation Week to attend to housing or other personal needs. Available housing options include:
On-Campus Residence Hall
Owen Graduate Center, located on campus, provides accommodations for graduate and professional students, post-doctoral fellows and research assistants. Housing fees for Owen Hall include accommodations, local telephone, and a limited amount for meals. Both single room and double room occupancy is available. Occupancy is limited to single students. Additional information on housing options for graduate students can be found on the Housing Services website.On-Campus University Apartments
The University owns and operates several apartment complexes that are available to MSU students. For detailed information about University Village, Spartan Village, and Cherry Lane apartments, visit the University Apartments website.Off-Campus Housing
East Lansing and the surrounding area provide a wide variety of living options from apartments to townhouses to single rooms, many within a few minutes' drive from campus. Most medical students choose to live off–campus. If you have a vehicle, the Lansing, Haslett, Okemos, and Williamston communities are within reasonable proximity to MSU. Be sure to indicate that you are a medical student when you contact apartments and housing agents. Post a roommate or apartment request or look for a roommate or apartment on the Housing/Roommate Match-Up Service website. This site is provided solely for the use of students enrolled in the College of Human Medicine, the College of Osteopathic Medicine, the College of Veterinarian Medicine, or the College of Law. When posting a request, using your MSU email address for your contact information is preferred. You must indicate within the text of your request that you are a CHM student or your post will be deleted.
Housing Purchases
All students should expect to relocate at the end of Year I or Year II to one of the clinical campuses, except in cases of documented hardships. Students are strongly advised, therefore, not to purchase a home or expect to otherwise "settle" in the local area.
VI. MSU Vehicle Registration and Parking Permits
Beginning July 30, 2008, you will be able to register your vehicle and obtain a Lot 100 parking permit online (for the combined Fall and Spring semesters combined only) at the Department of Police and Public Safety (DPPS) web site. You may also come in to the Parking Office to purchase a permit for Fall semester only. Opting to purchase your permit online will allow you to avoid the long lines at the Parking Office. Permit privileges are not valid until the permit is properly affixed to your windshield, so be sure to allow enough time for the mailing of permits ordered online. All vehicles operated or parked on MSU property by MSU students must be registered with the DPPS Parking Office.
Visa, MasterCard, Discover, American Express, and electronic checks are accepted as forms of payment when you register online. If you prefer to pay in cash you may come in to the Parking Office beginning July 30, 2008. Bring your student ID and your current vehicle registration.
You must register your vehicle based on your residency. The permit to register your vehicle will allow you to park either in Lot 89 (NW corner of Farm Lane and Mount Hope Road) or in the lot determined by your residency. You may also use pay Lots 62W, 79, 100 and RIII (Wharton Center) and pay the hourly rate.
If you want to park in Lot 100 (just east of the Clinical Center), you have two options:
Pay to register your vehicle for the entire year and pay the hourly rate on a daily basis. The cost to register your vehicle will vary depending on where you will be living.
Pay one flat rate per semester for a Lot 100 parking permit (to avoid paying cash on a daily basis). The flat rate is in addition to registering your vehicle (see #1) and does not include a gate card deposit. The deposit is refundable once you return the gate card to the MSU Parking Office. The gate card allows you access to Lot 100 without having to stop for the booth attendant. If you opt to pay the flat rate but not to obtain a gate card, you will have to stop and show your permit to the booth attendant whenever you exit the lot.
