the MD/PhD Application

The deadline for submission of the MD/PhD application is November 15, 2009. Be sure to keep a copy for your records.

The MD/PhD Application is not compatible with older versions of Adobe Reader.  Please be sure that you have Adobe Reader 7.0, at minimum, installed.

We recommend that you print a copy of your application for your records.

Submitting the MD/PhD Application

If you already have a default mail client set up on your computer, you should only need to click the SEND button in your email program after you've clicked the Submit button. 

To submit the application using a desktop email application (Outlook, Outlook Express, Windows Mail, Thunderbird, etc.):

  1. After you have completed the application, click the Submit button at the bottom of the application.

  2. In the Select Email Client dialog box, select Desktop Email Application; then click OK.

  3. Your default email application displays a new email message with the To, Subject, Body, and Attachment fields automatically filled in.

  4. Use your email application to send the email.

  5. Click Close in the Email Confirmation dialog box in Acrobat.

To submit the application using a web-based email service (Gmail, Hotmail, Yahoo!Mail, etc.):

  1. After you have completed the application, click the Submit button at the bottom of the application.

  2. You will be prompted to save the file to your computer. Save the file using the default file name (2010_MDPhD_application.pdf).

  3. Open a browser window, log into your web-based email service, and use your service to create a new blank email.

  4. Address the email to ecrocket@msu.edu. Give the message the subject line "MD/PhD Application."

  5. Use your web email service to attach the PDF file (that you saved in step 2) to your email message.

  6. Send the message.

If you are still unable to submit your application by email using the methods above, print your application using the Print button and then fax it to Dr. Crockett at (517) 432-9471.
